By submitting this form, I declare that Business Switch Pty Ltd (ABN 83 134 235 304) (Business Switch), is authorised to cancel my ABN on my behalf (as my registered tax agent for the purpose of actioning this cancellation request).
- I am the ABN holder, or I am an authorised representative of the ABN holder, and am authorised to submit this cancellation request to Business Switch.
- If cancelling the ABN, that all lodgement, reporting and payment obligations for all relevant government agencies have been met and associated processes completed.
- By cancelling the ABN, I understand that the following registrations connected with the ABN will also be cancelled:
- Goods and Service Tax;
- Pay As You Go;
- Luxury Car Tax; and
- Wine Equalisation Tax.
- I understand that cancelling the ABN will also cancel any Fuel Tax Credits attached to the ABN.
- I understand that once the ABN has been cancelled, the ABN holder will be removed from Business Switch’s Australian Taxation Office portal.
- If I have elected for Business Switch to cancel a business name attached to the ABN, that Business Switch is authorised to act as my ASIC agent and submit the cancellation request to ASIC.
- I understand that Business Switch will utilise information supplied by me on this form to request the cancellation of the registrations.
- I confirm that I have received the consent of any individuals referred to on this form for the submission of their personal information contained in this form to Business Switch and to the ATO and ASIC, as necessary to process the cancellation request(s).
- I verify that the information provided on the form is, to be the best of my knowledge, accurate and complete.
